    Fertility Tests for Females:

    • AMH: AMH test is one of the important and primary tests which need to be done prior to reaching a decision regarding the right fertility treatment. AMH stands for Anti Mullerian Hormone test. A good level of AMH indicates that the woman has a good ovarian reserve. If the woman has AMH levels <0.75 ng/ml then IVF + Donor egg is indicated. This is because if AMH level is low, then the ovarian reserve is also low and it requires an egg donor for the successful fertilization and formation of a healthy embryo. Usually an AMH between 1.5 ng/ml and 4 ng/ml indicates for an IVF treatment.
    • HSG: HSG test stands for Hysterosalpingogram test. It is a basic test utilized in the examination of Fallopian tubes patency and the shape of the uterine cavity. Here, a contrast dye is injected through a tube inside the uterus cavity and fallopian tubes. Then, x-rays are taken. This helps in determining any kind of presence of fibroids or cysts in the uterus or fallopian tubes. It also helps in determining whether any of the tubes are blocked or open.
    • Thyroid: Thyroid gland increases by 10% during pregnancy and the levels of T3 and T4 hormones increases up to 50%. Therefore, TSH levels should be low in the case of a successful pregnancy. If thyroid levels of a woman are <5 mIU/L then no treatment is needed. If thyroid levels are greater than 5 mIU/L, thenIVF may be suggested.
    • Ultrasound: In the ultrasound imaging test, the doctor uses a transducer to ensure that the reproductive organs of a woman are clearly seen. To detect PCOS, number of ovaries, no. of fallopian tubes, cysts in fallopian tubes and the follicles size, pelvic ultrasound is done.

    Fertility Tests for Males:

Semen Analysis Test: Semen Analysis is one of the most important tests which is performed for the successful analysis of several parameters of sperms which include:

    • Sperm Count or Sperm Concentration
    • Sperm motility (Movement of sperms)
    • Sperm Morphology (Shape and size of sperm)
    • pH of sperms

    Female Infertility Issues:

Females can have difficulty in conceiving when they experience abnormalities or fertility issues in their major reproductive organs such as ovaries, fallopian tubes and uterus. But with the help of assisted reproductive technology, it is now possible to conceive a child even with infertility issues. The complete diagnosis is based on the following profiles for a female:

    Ovarian Profile

Ovarian Profile tells the fertility score of the ovaries. Following parameters are checked to determine the overall health of the ovaries. If any of the parameters have some disorders, the couple may require treatment to complete their family.

    Number of Ovaries
In general, every female has two ovaries. Now, there can be cases when a woman has single or no ovary. In such cases, the couple requires treatment to conceive a baby.
    Number of OvariesDiagnosis TestReasons
1Pelvic UltrasoundGenetic or ovarian removal surgery
0 (Both ovaries are absent)Genetic or ovarian removal surgery
    Follicle Size (mm)Diagnosis TestReasons
    >20mmNormal functioning of ovaries and hormones
<20mmFollicular Study or UltrasoundHormones such as estrogen, FSH and LH are imbalanced. Stress can also result in low follicular size.
    Follicle Size
A woman is born with millions of follicles in her ovaries but only 300-400 number of follicles develop into mature eggs. The follicles start to mature when the woman reaches her puberty. And each month a single mature egg is released from one of the ovaries. Now, the egg quality declines significantly with age. If egg quality is poor, then the ovulation may not happen at all. If ovulation happens with an immature egg, it will not lead to the successful fertilization. Hence, egg quality or follicle size should be good for the successful conception.
    Egg Quantity or Ovarian Reserve
If the ovarian reserve is very low or diminished, it indicates the woman can no longer produce mature eggs. Egg quantity decreases with age. But sometimes, a younger woman can also have lower ovarian reserve due to a number of reasons.
    Ovarian ReserveDiagnosis TestReasons
    Very High (AMH > 4 ng/ml)PCOS
NormalAMHAll hormones are balanced and the ovaries are functioning properly
Low / Very Low (AMH < 1.5 ng/ml)Chromosomal abnormality, chemotherapy, ovarian cysts, ovarian tumor, endometriosis

Complex and smallLaparoscopyComplex cysts can be formed due to PCOS or an ovarian tumor. In case of larger and complex cysts, biopsy needs to be performed to rule out cancer
    Complex and large
    Polycystic Ovaries (PCO)
PCOS is Polycystic Ovarian Syndrome. It mainly involves three features which include high male hormone levels, multiple cysts in ovaries and missed or irregular periods. The estrogen and progesterone levels are low then the required levels and the androgen levels are much higher. This leads to an imbalance in the female and male hormonal levels in the body of the woman having PCOS.

    Fallopian Tubes Profile

Fallopian tube profile helps in determining the overall health and functioning of the fallopian tubes. When Fallopian tubes are blocked, then the transportation of eggs is hindered and this either prevents the occurrence of successful pregnancy or it leads to ectopic pregnancy. In the case of Fallopian tube blockage, the gynecologist or infertility specialist performs laparoscopic surgery for removing the blockage of the Fallopian tubes successfully.

    No. of fallopian tubes
Both the fallopian tubes should be functional for the natural conception. If any of the tubes has problems then chances of natural conception are reduced significantly.

    No of blocked fallopian tubes
Both the tubes should be patent or open so that the egg released from the ovary can meet sperms in case one someone is trying to conceive.
    Cysts in fallopian tubes
Cysts in fallopian tubes can lower the fertility levels in women. Along with causing fertility issues, cysts can also block the tube sometime.

    Uterus profile

Uterus is the main reproductive organ in the female where baby stays during pregnancy. Uterus profile helps in determining the overall reproductive health of the uterus.

    Size of the uterus
There are different types of uterus depending upon the shape, position and the size. Abnormal shape and size suggest fertility treatment.

    Endometrium Lining
Endometrium lining is an important part of the lining of the uterus which comprises of three main layers. Endometrium lining sheds in the times of menstrual cycle and it is restored in the case of pregnancy. In case of a positive pregnancy and absence of menstrual cycle, endometrium lining attaches and joins with the other layer of uterus known as myometrium. The levels of progesterone increase in case of pregnancy which helps in thickening the endometrium lining thereby providing a solid guard to the growing baby.
    Presence of fibroids
Fibroids are the non-cancerous growths which are present in or around the uterus of a woman. Some of the common symptoms which a woman with fibroids can face include heaving bleeding during menses, long duration menses, pain or feeling pressure in the lower abdomen, pain in the lower back. It can be detected with the help of Ultrasound and pelvic MRI.

In case of the presence of fibroids, fertilization of eggs and sperms is compromised to a certain extent. Also, the implantation of the embryo in the uterus is hindered due to a decrease in the normal size of the uterus.

Prolactin hormone is responsible for milk production after pregnancy delivery. But sometimes due to certain disorders non pregnant women can have higher levels of prolactin which indicates infertility.
    Age Factor
Age of the female partner has a direct impact on the fertility levels. With increasing age, egg quality and quantity deteriorates a lot and fertility treatments are required to complete one’s family. Chances of miscarriage and stillbirth also increase with increasing age.

Proper diagnosis and timely treatment can help aged couples in completing their family successfully. Know when should you visit a doctor:

Female age (<30 years) : If a couple tries actively to conceive a child for more than 1 year and still unsuccessful, in such cases infertility evaluation is recommended
Female age (30 - 40 years) : If trying to conceive for more than 6 months, then consult a Reproductive Endocrinologist to complete infertility evaluation
Female age (>40 years) : Female fertility levels drops remarkably after 40 years of age. In such cases, it’s better to consult a gynecologist before trying to conceive.

    Male infertility issues

Whenever a couple faces problems in conceiving, it can be due to either male or female infertility. Male fertility levels can be checked by semen analysis test where certain semen parameters are checked to determine the overall quality of semen.

    Semen Profile
Semen profile helps in determining the overall reproductive health of the males. It includes checking various semen parameters before deciding the course of treatment.
    Sperm Count
Sperm count is the number of sperm present in the semen of a man. The sperm count which is considered to be normal is 50 million sperms. If the sperm count is less than 50 million, then the condition is known as oligospermia. If the sperm count is NIL due to obstructive azoospermia then the different course of treatment is followed.

Q1. Which are the important reproductive organs in females and males?

The important reproductive organs of females include :

    • Ovaries- Ovaries are reproductive glands which produce eggs for fertilization with sperms. Ovaries also produce estrogen and progesterone hormones. Both the ovaries should have good ovarian reserve and quality of eggs to conceive a child naturally.
    • Fallopian Tubes- Fallopian Tubes are uterine tubes that carry eggs from ovaries to the uterus of a woman every month. In case, sperms are present during the time of egg release, fertilization takes place in the fallopian tube. After successful fertilization, the embryo implants itself into the lining of the uterus.
    • Uterus- Its basic function is to shield the embryo and provide the baby to grow peacefully in the mother’s womb for nine months. Uterus lining should be optimum to support nutritional requirements of the baby during pregnancy duration.

The important reproductive organs of males include

    • Testes-Testes or testicles store sperm. They also produce testosterone hormones.
    • Epididymis-It is one of the crucial male reproductive organs present in the form of thin tubes which carries sperm present in the testes to ductus deferens (vas deferens) present in a male’s body. If epididymis is blocked, it can result in azoospermia.
    • Vas Deferens-Vas deferens (ductus deferens) is a muscular tube which carries sperm to ejaculatory duct from Epididymis
