Infertility is progressively becoming a common problem, affecting couples across the globe. After facing failure in attempts to get pregnant in a natural way, many infertile couples turn to IVF to have better chances in conceiving. IVF and ICSI are among the most recognized assisted reproductive techniques, and this advanced procedure has helped many thousands of couples achieve their dream of having a baby. However, for men with severely reduced levels of sperm morphology or those with DNA damaged sperms, there is an increased risk of miscarriage and reduced clinical pregnancy rates, even with ICSI treatment. With increasing male infertility cases and risk of miscarriage, these techniques are becoming inefficient in providing the best results. IMSI centres in India give a completely new meaning to IVF treatment as IMSI or Intracytoplasmic Morphologically-selected Sperm Injection technique allows analysis of sperms under ultra-high powered magnification. Healthy motile sperms with the best morphology is manually selected and microinjected into the egg. This improves the chances of fertilization and reduces the risk of miscarriage.
With IMSI technique, IMSI Centres in India have better access to the structure of the sperms that allows embryologists to look at the sperms in detail. This is done using a digital imaging microscope that can provide magnification up to 6,000 times to examine sperms to help in the selection of the quality sperms with most normal morphology thereby excluding the ones with suspected structural alterations and abnormality. It is a real-time method where healthy motile sperms are manually picked before the microinjection takes place.
IMSI technique is performed to diagnose male fertility problems, such as low sperm count or low sperm motility. It is also recommended when there is no sperm in the semen or if the patient had a vasectomy. It also comes into play for surgical removal of sperms from testes. Using IMSI technique, the embryologists can select better quality sperms, which can result in higher pregnancy rates as well as lower miscarriage rates compared to ICSI. In IMSI Centres in India, embryologists use pre-defined sperm sizes, shape and quality to select the most appropriate sperm to be injected into the egg.
IMSI centres in India normally use this technique in reproductive laboratories to carry out ICSI procedures as it allows the embryologist to detect subtle structural alterations in sperm and select spermatozoa with the most normally shaped nuclei.
IMSI technique is adopted by IMSI centres in India as it provides significant chances of fertilization, high pregnancy rates, and reduction in miscarriage rates. Sperms with normal morphology and healthy structure are picked individually and injected into an egg for fertilization. This method is highly recommended to patients who undergo surgical sperm retrieval treatments like MESA (Microepididymal Sperm Aspiration) or TESE (Testicular Sperm Extraction). It offers the highest success rate in male infertility cases as only the best quality sperms are used and the rest is discarded.
Male infertility is determined by analyzing a sperm sample. The sperm is tested for its quality, motility, and morphology. Abnormal results in any of these parameters can be the cause of infertility. Depending on the severity of the case, an infertility specialist might recommend IUI (Intra-Uterine Insemination) or IVF combined with ICSI treatment.
Unfortunately, the ICSI technique, in which, the sperm is artificially injected directly into the egg, is not as effective as one might hope and has a rather high miscarriage and low fertilization rate. IMSI allows precise selection and introduction of the healthiest sperm into an egg, resulting in higher chances of a pregnancy compared to other fertility techniques.
Reasons to Choose IMSI Treatment in IMSI Centres In India:
IMSI follows the same procedure as ICSI, but an extra step of viewing sperms under a high-magnification microscope is added to improve the selection of quality sperms before injection. This has vastly made a difference in the fertilization and pregnancy rates. IMSI is recommended for the below cases:
For patients that have above-mentioned conditions, IMSI Centres in India offers Intracytoplasmic Morphologically-Selected Sperm Injection (IMSI) treatment. At these centres, an andrologist first examines the male partner for the possible causes of infertility and follow regulated procedures to obtain the best sample of semen. This is followed by IMSI where an embryologist conducts the procedure in special contained laboratories using high-magnification digital imaging microscope. They enlarge the images of the sperms up to 6000 times and look for the most appropriate sperms with best health and structure that can fertilize an egg and possibly result in pregnancy.
Sperms are normally retrieved through masturbation but in most infertility cases, the male partner is unable to produce semen or has the extremely low number of sperms in the semen during ejaculation. In these cases, sperms are surgically retrieved from testicles or epididymis. The sperms retrieved are washed and prepared for analysis under a microscope.
The egg retrieval process is simultaneously performed with IMSI to begin ICSI treatment as soon as possible. The female partner is given a series of hormonal injections to stimulate egg maturation in the ovaries. She is kept under strict inspection for some days to monitor her body’s response to medication. Subcutaneous injections continue for approximately 36 hours before the eggs are ready to be retrieved. At that point, the woman is given a trigger shot injection to prepare the eggs for maturation.
Eggs are retrieved from ovaries when they are healthy and mature for fertilization. A surgical procedure called transvaginal ultrasound aspiration is performed under conscious sedation where a suction device connected to a needle is used to remove multiple mature oocytes from the follicle at a time. Sometimes the patient is also given oral promethazine to prevent nausea during the procedure. After the egg is retrieved, the women generally remain in the clinic for one to two hours and undergo a general follow-up exam. To prevent infection the donor is given antibiotics such as oral doxycycline and in some cases; ultrasound is conducted to confirm the health of the patient.
Following egg retrieval and IMSI, the embryologists perform ICSI treatment where the selected sperms are individually injected into each egg. After the eggs are fertilized, a required number of healthy embryos are transferred into the woman’s uterus.
After microinjection, the oocytes are washed and transferred into the Fertilization Medium. The embryologist examines the egg for signs of fertilization the day after the sperm is injected into each egg. Embryos are transferred at any stage from the second day to the sixth day after the initial procedure. The embryos in right conditions are expected to reach the blastocyst stage by day five or six. The fertilized egg or embryo is transferred inside the uterus, three to five days after fertilization. At IMSI centres in India, the entire procedure is conducted in a sterile, fully equipped lab following stringent medical protocols to improve chances of pregnancy.
Embryologists use a fine catheter to transfer the embryo into the uterus and wait for pregnancy. A pregnancy test is conducted after two weeks of embryo transfer process. People are anxious during the first two weeks with a concern that they might do something wrong which can lead to the failure of the treatment. To keep worries aside, here are some precautions patients must follow after embryo transfer:-
With increasing male infertility cases in India, couples are looking for advanced treatment procedures for a better chance to conceive. The city is also witnessing enormous growth in medical tourists due to the availability of competitive treatment procedures at cost-effective fees. IMSI is one of the new techniques, which are being evolved and developed at par with international standards. Since the introduction of Intracytoplasmic Morphologically-Selected Sperm Injection, IMSI centres in India have witnessed a rise in pregnancy rates from 30 percent to 66 percent.
Today, India stands at the forefront of reproductive treatments as along with common infertility treatment methods, new methods like those that IMSI is also practiced in the facilities. IMSI centres in India have given hope to many childless couples and unlike before, seeking medical help in infertility cases is no longer considered a taboo.
AT IMSI Centres in India, the cost of IMSI is between Rs. 1, 50, 800 to Rs. 2, 76, 500 in addition to IVF treatment costs. This additional cost represents the use of the highly sophisticated technology and the dedicated time taken by embryologists to perform IMSI with IVF. As the success rate of IMSI is comparatively higher than ICSI treatment, similarly the treatment cost of IMSI is also higher than ICSI.
During a study, it was observed that employing IMSI technology increased the success rates from 33 percent to 66 percent. IMSI involves microinjecting only the healthy and morphologically selected sperms into an egg, it significantly improves the embryo quality and development without affecting the chromosomal status of embryos. Therefore, IMSI success rate is believed to be higher than conventional ICSI. However, the success rate also greatly varies on the type of case and the care it receives.
